• •Location: Vasona Creek Healthcare – Los Gatos/South Bay Area
• •Pay Rate: $62.00–$72.00+ per hour, DOE
• •Opportunities: Full-Time, Part-Time & PRN

Your Role & Impact

As a Physical Therapist, you will play an essential role in helping patients achieve their highest level of function, independence, and quality of life.

Responsibilities include:

• •Perform comprehensive physical therapy evaluations and develop individualized, evidence-based treatment plans.
• •Provide skilled therapy services to short-term rehabilitation and long-term care residents.
• •Treat patients recovering from joint replacements, cardiac events, strokes, and other medical conditions.
• •Utilize specialized rehabilitation programs, including Acute Neuro Rehabilitation and Pain Management .
• •Monitor patient progress and modify treatment plans as appropriate.
• •Collaborate with PTAs, OTs, SLPs, nursing, and other members of the interdisciplinary team.
• •Educate patients and families regarding treatment plans, exercises, mobility, and strategies for continued progress.
• •Promote safe mobility, functional independence, and successful discharge planning.
• •Maintain accurate and timely clinical documentation.
• •Advocate for patients and support their individual rehabilitation goals.

Qualifications

• •Current California Physical Therapist (PT) license required.
• •Previous experience in skilled nursing, post-acute, or rehabilitation settings preferred.
• •Strong clinical reasoning and communication skills.
• •Passion for rehabilitation and helping patients achieve meaningful functional outcomes.
• •Commitment to evidence-based, patient-centered care.
• •Ability to work collaboratively as part of a multidisciplinary rehabilitation team.
