As a Physical Therapist , you’ll contribute to our success in the following ways:

• •Evaluate patients and develop individualized treatment plans.
• •Provide physical therapy services in the home setting.
• •Monitor patient progress and adjust care plans as needed.
• •Collaborate with physicians, caregivers, and the interdisciplinary care team.
• •Complete timely and accurate clinical documentation.
• •Supervise therapy assistants and support staff, as appropriate.

What is Required?

• •Graduate of an American Physical Therapy Association (APTA) approved physical therapy program
• •Current, unrestricted PT license valid for the state of work
• •Minimum one (1) year of experience in a healthcare facility
• •Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• •Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
