This position is a temp position that will travel to multiple locations in Rural South King County (Kent, Auburn, Federal Way, Des Moines, Covington).

Summary of Responsibilities:

• •Provide physician orders and plan of care under the direction and supervision of the Director of Clinical Services.
• •Work in conjunction with all other home care personnel in patient care planning.
• •Direct and supervise Physical Therapy Assistant and Home Health Aide, as assigned.
• •Take initial patient history and make initial evaluation.
• •Consult with physicians regarding change in treatment.
• •Instruct patients and family in home programs and activities of daily living.
• •Participates in Quality Assessment and Performance Improvement activities as assigned.
• •Complete and submit OASIS assessments, reassessments, transfers, resumptions of care, discharges and significant change in condition.
• •Appropriately utilizes ICD-10 codes.

Requirements

• •Graduate from a physical therapist curriculum approved by the American Physical Therapy Association.
• •Currently licensed as a physical therapist in the state of Washington.
• •Two (2) years' experience as a physical therapist, preferred.
• •Home Health experience, preferred.
• •Detail oriented, self motivated and capable of efficient time management.
• •Critical thinker capable of working collaboratively with a team.
• •Valid WA State driver's license.
