What You’ll Do

As a Physical Therapist in a Skilled Nursing Facility , you will help patients regain mobility and independence following illness, injury, or surgery.

Key Responsibilities:

• •Evaluate patients and develop individualized treatment plans
• •Provide evidence-based physical therapy interventions
• •Collaborate with physicians, nurses, and rehab team members
• •Supervise Physical Therapy Assistants (PTAs)
• •Document patient progress accurately and timely
• •Participate in care planning and interdisciplinary meetings
• •Educate patients and caregivers on mobility and safety
• •Support discharge planning and functional outcomes

Requirements

Qualifications

• •Graduate of an accredited Physical Therapy program (DPT, MS, or BS)
