​Job Description:

We are seeking a compassionate and experienced Primary Care Physician Assistant to join our dynamic team at Foundation Medical Partners / Southern NH Health. In this role, you will play a vital role in providing comprehensive care to a diverse patient population in Nashua, NH .

• •Collegial practice with solid APP support

• •40 Hours – 50% direct patient care work and 50% coordinating and communicating patient care

• •Hybrid Work Schedule – Potential remote options once fully trained

Qualifications:

• •Certified Physician Assistant
• •2+ years of Primary Care experience
• •Strong clinical skills, including assessment, diagnosis, and treatment planning.
• •Excellent communication, collaboration, and interpersonal skills.
• •Proficiency with EPIC EMR preferred
• •Ability to work independently and as part of a team.

Work Shift:

Mon-Friday 8-5, (With potential for hybrid remote work)
