Description
We are seeking a compassionate and skillful Internal Medicine physician, with a proven track record for success in chronic disease management, and a passion for increasing health care access and outcomes for underserved communities.

The position is at our new health center in Fullerton, Califnoria. The facility is currently under development, and the selected clinician will serve at a nearby site until the new site is complete and ready for patients.
More details on the position :
- Provide high quality and evidence-based primary care to underserved patients. Most patients will be individuals recently discharge from local hospitals that don't have an established medical home.
- Using a team-based model of care, collaborate with other caregivers to effectively improve the overall health of our patients and communities.
- Responsible for performing physical and psychosocial assessments, diagnosis and management of chronic and acute health and illness needs.
- Assumes responsibility and accountability for the continuity of healthcare and health education regardless of the presence or absence of disease.
- Perform in-clinic procedures
- Utilizes standardized procedures
- Consults with Chief Medical Officer on complex cases, as needed.
- Supervise advance practice nurse or PA, as assigned.
- Ensures services are evidenced-based, trauma-informed, culturally attuned, and tailored to patient population needs.
- Documents, via EMR, all patient interactions, treatments plans, and interventions utilized in a timely manner.
- Assist with coverage and tasks of other providers, as needed.
- Perform care and services confidentially, in accordance with local, state, and federal laws.
- Participate in case consultations, professional development opportunities, as needed and guided by department and patient needs.
- Adhere to department and organizational programs.
- Other duties, as assigned.
Benefits
Salary:
$253,000 - $289,000, Yearly based on a 40-hour work week.

Requirements
- Education:
- Doctorate in Medicine from accredited institution
- License / Certifications:
- Licensed MD/DO in the State of California
Board Certified/Eligible in Internal Medicine
DEA certified
Provider level basic life support (BLS) from American Heart Association approved program
California Medi-Cal and/or PAVE enrolled provider
- Experience:
- Experience providing direct patient care. preferably in an outpatient or community-based setting
- Training:
- Completion of an accredited residency program in Internal Medicine.
Knowledge/Skills/Abilities:
- Proficiency in EMR Systems, with EPIC experience preferred
- Demonstrated ability to read, write and speak English fluently
- Willingness to use a translator or translation services to best serve patient treatment needs
- Proficient in computer programs and systems, including email, internet, and Microsoft Office suite
- Expertise in applying evidenced-based medicine to treat and diagnosis. Demonstrated ability in applying methods of scientific inquiry and exercising of clinical judgment in challenging situations.
- Exemplar communication and leadership skills, including active listening, taking initiative, following guidance, and setting examples for excellent care of patients.
- Commitment to continuous learning and professional development, remaining current on medical practices and professional credentialing and licensure necessary for continuous employment with St. Jude Neighborhood Health Centers.
- Ability to interface effectively with all caregivers across all levels and departments.
- Demonstrated commitment to interpersonal and cultural sensitivity, valuing diversity, equity, inclusion and belonging, promoting high-quality care to diverse populations.
- Demonstrates the ability to read, write and speak English and Spanish without an interpreter, is preferred.
