Job Description:

Up to a $10,000Sign on Bonus Available

• Schedule: Full-Time/Day
• Shift: Day Shift Monday-Friday 8am to 5pm with e/o weekend or call rotation.
• Location: Barington, Schaumburg, Batavia, Carol Stream, Hoffman Estates, Itasca, Medinah, Roselle, St Charles, and Steamwood Area with travel to LaGrange Hospital for meetings/Orientation etc.

Mileage reimbursement provided: $0.76 per mile

Flexible scheduling with Full Time, Part Time and PRN available

Company provided phone and laptop/iPad

• •Implements and monitors patient care plans to ensure effective treatment.
• •Monitors, records, and communicates patient condition as appropriate.
• •Serves as the primary coordinator of all disciplines for well-coordinated patient care.
• •Notes and carries out physician and nursing orders accurately and promptly.
• •Conducts holistic, individualized assessments of each patient considering age, physiological, and psychological status.
• •Administers medications and solutions as required.
• •Documents all phases of patient status during care.
• •Participates in the transfer of patient information to ensure continued quality care.
• •Leads and mentors staff to ensure optimal performance and competency in care delivery.
• •Maintains a safe and clean working environment by complying with policies, procedures, rules, and regulations.
• •Develops and maintains positive working relationships within the practice and other departments.
• •Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
• •Other duties as assigned

Education:

• •Associate's of Nursing [Required]
• •Bachelor's of Nursing [Preferred]

Work Experience:

• •1+ acute care clinical experience [Preferred]
• •1+ home health experience [Preferred]

Licenses and Certifications:

• •Registered Nurse (RN) [Required]
• •Basic Life Support - CPR Cert (BLS) [Required]
• •Driver's License (DL) [Required]
• •Auto Insurance (AL) [Required]
