Resident Care Coordinator – Assisted Living

Columbia Place Assisted Living & Memory Care is seeking an experienced and dependable Resident Care Coordinator (RCC) to join our Health Services leadership team.

This is a hands-on leadership position for someone who enjoys working with residents while also supporting and coordinating a team of caregivers and Med Techs. The Resident Care Coordinator works closely with our nursing and community leadership to help ensure residents receive consistent, high-quality care and that our Health Services department is appropriately staffed and supported.

A major responsibility of this position is managing the day-to-day Health Services schedule for our caregivers and Medication Technicians. We are looking for someone who is organized, responsive, comfortable solving staffing challenges, and willing to step in when the team needs support.

What You'll Do

  • Manage and maintain the caregiver and Medication Technician schedule, including coordinating requested time off, vacations, call-outs, and staffing changes.
  • Help ensure appropriate staffing levels are maintained based on resident needs and state requirements.
  • Provide on-call support for Health Services staffing needs and employee questions. This position participates in an on-call rotation and requires availability outside normal business hours when staffing or resident-care needs arise.
  • Provide leadership, direction, and day-to-day support to caregivers and Medication Technicians.
  • Assist with interviewing, hiring, onboarding, training, and development of new Health Services employees.
  • Work closely with nursing leadership on resident service plans, evaluations, changes of condition, appointments, and other resident-care needs.
  • Communicate resident changes and concerns to nurses, providers, families, and other appropriate individuals.
  • Assist with medication-room and medication-cart organization, supplies, and equipment.
  • Respond to resident emergencies and assist with appropriate follow-up.
  • Serve as a liaison between frontline Health Services employees and community leadership.
  • Help maintain compliance with infection-control, safety, and resident-care procedures.
  • Work as a Medication Technician and/or caregiver when needed to support the department, cover shifts, remain connected to resident-care needs, and provide hands-on coaching to staff.
  • Assist with additional Health Services responsibilities as assigned.

What We're Looking For

We are looking for someone who is dependable, organized, calm under pressure, and comfortable leading others. This position requires someone who can balance administrative responsibilities with the realities of operating a 24/7 resident-care department.

The ideal candidate is someone who doesn't simply identify a staffing or resident-care problem—they take ownership of it and help find a solution.

Qualifications

  • Minimum of 1 year of experience as a caregiver/Personal Care Attendant OR at least 6 months of experience as a Medication Aide/Medication Technician.
  • Assisted living experience strongly preferred.
  • Previous scheduling, lead, supervisory, or care-coordination experience preferred.
  • Strong organizational and communication skills.
  • Ability to communicate effectively with residents, families, employees, and leadership.
  • Ability to problem-solve and make sound decisions in a fast-paced environment.
  • Must maintain required certifications, including medication and CPR certifications as applicable.
  • Must be willing and able to provide direct resident care when needed.
  • Must be available for scheduled weekends and holidays.
  • Must be able to participate in the on-call rotation and respond to staffing needs outside regular business hours.
