Job Description:

AHMG Palliative Medicine and Supportive Care

2501 N. Orange Ave., Suite 235, Orlando, FL

Monday - Friday 8am - 5pm

position will support practices in East Orlando and Altamonte

• •Assesses patients' physical, psychosocial, cultural, and spiritual needs through observation, interviews, and review of medical records and clinical data.
• •Develops and implements patient care plans in collaboration with the interdisciplinary team, observing outcomes in accordance with nursing standards.
• •Demonstrates competency in therapeutic and diagnostic interventions, including IVs, wound care, medication administration, catheterization, and specimen collection.
• •Advocates for and protects the health, safety, and rights of each patient.
• •Provides patient and family education based on assessment of learning readiness, needs, and ability, and evaluate the effectiveness of teaching.
• •Encourages patient engagement in healthcare through motivational interviewing, self-management protocols, and teach-back methodology.
• •Delivers relationship-based care, engaging patients in all aspects of care with respect for cultural differences and diversity.
• •Delegates tasks to non-RN personnel in accordance with State Board of Nursing and hospital policy.
• •Coordinates and implements group visits for identified high-risk patient populations.
• •Aids in the care transition of high-risk patients, including medication reconciliation and coordination of services.
• •Contributes to continuous process improvement by actively participating in patient experience and satisfaction initiatives.

Pay Range:

• $29.50 - $47.20
• Background Screening Requirement (Florida Law)
