The RN Case Manager is responsible for providing nursing care to patients, including identifying and coordinating patient/family care with other members of the Interdisciplinary Team and health care providers.

RESPONSIBILITIES:

• •Completes an initial assessment of patient and family to determine hospice care needs; provides a complete physical assessment and history of current and previous illness
• •Uses health assessment data to determine nursing diagnosis
• •Triages calls and assists patient/primary caregiver with problem resolution
• •Teaches and counsels patients and families in meeting nursing and related needs
• •Makes visits for symptom management and support as needed
• •Modifies patient treatment plans as indicated by patients' responses and conditions
• •Maintains current, accurate, detailed reports in accordance with policy
• •Monitors, records and reports symptoms and changes in patients' condition to the IDG
• •Provides health care instructions to the patient as appropriate per assessment and plan
• •Obtains and receives physician orders as required
• •Pronouncement of patients

Requirements

• •Current license to practice as a registered professional nurse in Georgia
• •Current and continued CPR certification
• •At least one or two years of recent home care experience; hospice experience preferred
• •Good verbal and written communication and problem solving skills
• •Responsible, valid driver’s license; reliable, insured, automobile for making home visits required
