As an RN Case Manager, you will play a critical role in coordinating and delivering high-quality home health and hospice services while building meaningful relationships with patients and their families.

Responsibilities

• •Complete comprehensive patient assessments and develop individualized care plans.
• •Coordinate care with physicians, therapists, social workers, and other healthcare professionals.
• •Provide skilled nursing care in accordance with physician orders and agency policies.
• •Monitor patient progress and modify care plans as needed.
• •Educate patients and family members regarding disease management, medications, and plan of care.
• •Maintain accurate and timely clinical documentation.
• •Participate in interdisciplinary team meetings and case conferences.
• •Ensure compliance with federal, state, and agency regulations.

Qualifications

• •Current Idaho RN license in good standing.
• •Graduate of an accredited nursing program.
• •One year of nursing experience required.
• •Home health or hospice experience preferred.
• •Strong assessment, communication, and organizational skills.
• •Ability to work independently and effectively manage a caseload.
• •Current driver's license, reliable transportation, and auto insurance.
