This position plays a vital role in ensuring patients receive timely, appropriate care by assessing symptoms, prioritizing needs, coordinating follow-up, and serving as a key clinical resource for patients and providers.

What You'll Do

• •Conduct telephone and electronic triage for patient concerns and symptoms.
• •Assess urgency of patient needs using nursing judgment and clinical protocols.
• •Provide patient education and self-care instructions.
• •Coordinate care with providers and clinical staff.
• •Process medication refill requests according to clinic procedures.
• •Review and communicate laboratory and diagnostic results as directed.
• •Schedule patients appropriately based on clinical need.
• •Document all patient interactions accurately in the electronic health record.

Requirements

• •Active RN license.
• •Current BLS certification.
• •Minimum one year clinical nursing experience.

Preferred:

• •Ambulatory care, family practice, internal medicine, urgent care, or triage experience.
• •Experience with electronic health records.
• •FQHC or community health center experience.
