Help individuals and families build confidence, skills, and stability as a Self-Sufficiency Case Manager. Join our team and be a source of advocacy and support!

• WHAT YOU'LL DO:
• •Empower & Support: Provide case management services that help individuals and families set and achieve personal goals.
• •Guide Participants: Conduct assessments, develop individualized plans, and assist in stabilizing housing situations.
• •Manage Cases: Keep detailed records, document all contacts, and maintain participant confidentiality.

Promote Self-Sufficiency: Support participants in becoming independent from public assistance.

• •Community Engagement: Build relationships with service providers, attend networking meetings, and advocate for participants.
• •Stay Organized: Ensure accurate documentation, track referrals, and complete necessary reports on time.

• •You can assess needs and create action plans that drive real results.
• •You have experience working with multiple population types and advocating for individuals.

• EDUCATION AND EXPERIENCE:
• •Minimum of two (2) - four (4) years of education and/or experience in a related field required.

A four-year post-secondary degree in Human Services in preferred.
