SUMMARY As a Certified Nursing Assistant, you will provide high quality, individualized patient care in collaboration with the nursing team. Your ability to take initiative while always assisting and maintaining patient care will be critical in the success of the entire team.

RESPONSIBILITIES

• •Patient Care & Service. Provide and/or assist the resident in activities necessary as per individualized care planning; transport to and from activities as needed; provide leisure activities as needed; perform evaluation and reporting; provide resident and family with emotional support
• •Safety. Assist in maintaining a clean and safe resident environment
• •Communication & Documentation. Support families, direct them to appropriate professional staff; respond to resident requests in a timely manner; receive/give reports to team members/team leaders

REQUIRED QUALIFICATIONS:

• •Completion of NYS approved certified nursing assistant program
• •Minimum age requirement: 18
• •Registered Certified Nurse Aide in New York State

PREFERRED QUALIFICATIONS:

• •Ability to read, write and speak English, ability to follow verbal and written instruction
• •Associates degree

CNA - Certified Nursing Assistant - NYS Office of Professions, NYS DL - valid New York State Driver's License - New York State Department of Motor Vehicles (NYSDMV)

PHYSICAL REQUIREMENTS:

Drive - Clinical - Roles in this category require frequent driving combined with mobile clinical care across settings such as emergency response, mobile clinics, and home health environments. Staff must repeatedly enter and exit vehicles, occasionally lift and carry equipment weighing up to 100 lbs. or more, and perform care within patients’ homes or other confined, unpredictable, or uncontrolled environments. These positions demand strong fine and gross motor control, firm grasping, and heightened visual and auditory awareness to ensure safe and effective care delivery. Emotional and cognitive demands are substantial due to rapid decision‑making, direct exposure to critical or sensitive situations, and independently managing patient care in the field or home setting. Environmental exposures may include traffic hazards, infectious agents, extreme temperatures, variable home conditions, and hazardous materials. Valid licensure, physical readiness, and flexibility for extended or on‑call schedules are typically required.
