Pay Range:

$47.50 - $71.25

• Position Highlights
• •Monday–Friday | 8:00 AM–5:00 PM
• •Shared call rotation
• •In-person and virtual visits supported by Medical Assistant
• •Manage complex patients with ongoing follow-up throughout stay
• •Collaborate with SNF team to facilitate care coordination
• •Implement discharge planning to improve patient outcomes
• •Signing bonus, CME, and relocation available.

Candidate Qualifications

• •Passion for working in a SNF/Post-Acute Care Environment
• •Experience in a Skilled Nursing Facility is preferred but not required
