We are building a team of professionals who want to create a safe, welcoming, trauma-informed environment where individuals experiencing substance use and co-occurring mental health needs can take meaningful steps toward recovery and greater independence.
Joining the program at its beginning offers a unique opportunity to help establish the culture of a new team and contribute to how services are delivered from day one. We are looking for people who value teamwork, communicate well, believe in treating individuals with dignity and respect, and want their work to make a meaningful difference.

Maintain clinical records and complete required tasks and documentation accurately and within agency-established timelines.
Develop and carry out appropriate discharge plans to support a smooth transition from residential treatment and continued access to needed services and resources.
Conduct post-discharge follow-up with individuals to assess continuity of care, confirm receipt of needed supports, and identify potential barriers.
Establish and maintain effective working relationships with community agencies, organizations, providers, and individuals.
Maintain current knowledge and a working database of community resources and service connections.
