SOZIALPÄDAGOGE/SOZIALPÄDAGOGIN (M/W/D) IN DER EINGLIEDERUNGSHILFE SGB IX

Entgeltgruppe S 11b TVöD-SuE

unbefristet│Teilzeit mit 19,5 Wochenstunden│Flexibilität erforderlich

Ihre Aufgaben

  • Fallmanagement von Eingliederungshilfen für Kinder und Jugendliche mit Be-hinderung oder von Behinderung bedrohte Kinder und Jugendliche gem. SGB IX/HAG
  • Ausgestaltung des individuellen, ressourcen- und zielorientierten Gesamtplanverfahrens (Teilhabeplan, Zielvereinbarung) zur Teilhabe am Leben in der Gesellschaft unter Beteiligung der Betroffenen sowie Dritter
  • Anwendung des Instruments B.E.Ni zur Bedarfsermittlung (Bedarfsermittlungs-instrument Niedersachsen)
  • Durchführung und Leitung von Fall-konferenzen sowie Hospitationen in Schulen und Kindertagesstätten
  • Entscheidung über die Notwendigkeit und Eignung von Eingliederungshilfeleistungen
  • Koordination der Leistungserbringenden
  • Fortschreibung und Evaluation von schriftlichen Leistungsabsprachen

Ihr Profil

  • Abgeschlossenes Studium als Sozialpädagoge/Sozialpädagogin bzw. Sozialarbeiter/Sozialarbeiterin (Diplom oder Bachelor of Arts) mit staatlicher Anerkennung
  • Möglichst fundierte Kenntnisse in den gesetzlichen Grundlagen des Bundes-teilhabegesetzes sowie über die Vorschriften des Sozialgesetzbuches
  • Erfahrungen in der Arbeit eines kommunalen Sozialdienstes sind von Vorteil
  • Gute IT-Kenntnisse und sicherer Umgang mit den Office-Programmen, dem Fachverfahren „Open Prosoz" sowie der digitalen Akte „Enaio" sind von Vorteil
