Hera is dedicated care management in the community for older adults, covered by Medicare. Every family gets matched with a dedicated Hero (care manager) who has the tools, time, and autonomy to deliver ongoing support. You're the clinical entry point—the comprehensive assessment that sets the clinical foundation for real, ongoing intervention.
The Role
We're building the clinical infrastructure that makes great geriatric care exceptional. As a Clinical MD, you'll conduct telehealth assessments that serve as the clinical gateway for families newly enrolled with Hera. Each 45-60 minute visit establishes the clinical foundation: confirming Medicare eligibility, synthesizing complex medical and social history, identifying barriers to treatment, and flagging clinical priorities for our care managers (Heroes) to address over months of ongoing coordination. This assessment work directly shapes the care strategy that families receive.
Key Responsibilities
Conduct comprehensive telehealth assessments for newly enrolled Medicare clients
Synthesize complex medical histories and identify clinical priorities for ongoing care coordination
Document clinical findings that inform care manager (Hero) intervention strategies
Provide clinical oversight and collaboration as needed with care management team
Establish clinical credibility and trust during initial patient contact

Requirements
MD or DO (must be licensed to practice in all 50 states)
Board certification in family medicine, internal medicine or other relevant speciality.
Substantial experience in geriatric medicine, primary care, or chronic disease management
Confident in telehealth assessment and skilled at building clinical rapport across a screen
Strong clinical judgment and comfort with complexity and ambiguity
