JOB SUMMARY:

Functions as a primary care giver of a resident/client in their home under the direction and supervision of a registered nurse. Sees patients on a regular basis to deliver health care directly.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

(Other duties may be assigned.)

§ Assesses patient’s needs on an ongoing basis.

§ Assists in planning treatment/intervention and evaluation effectiveness of medications and treatments and reports such to RN and MD.

§ Provides assigned nursing care under the direction and supervision of a registered nurse in accordance with professional standards and pertinent rules and regulations.

§ Prepares and records notes for the clinical record.

§ Performs assigned treatments, including filling medicine boxes for patients, administration of medications, injections and procedures in accordance with physician orders and the plan of care.

§ Communicates with the physician regarding medication changes, tests, or other orders or changes in patient status.

§ May assist resident in emergency situations; notify the doctor, family and arrange transportation, as required.

§ Makes every effort to listen to and understand all of our customers (including internal and external customers, co-workers, patients, and family members), anticipating customer needs, and giving high priority to customer satisfaction.

§ Utilizes appropriate and effective interpersonal communication skills specific to job responsibilities.

§ Performs all job responsibilities with a friendly, positive and team oriented attitude.

§ Participates with special projects and/or overtime as requested.

§ Had knowledge of the organization, functions and services of the agency.

§ Protects the integrity of the organization, patients and co-workers by maintaining confidentiality of all patient and business information.

§ Maintains a safe work environment to minimize risk to patients and staff by complying with all safety/risk management policies and procedures.

§ Maintains and contributes to the efficiency of operations by consistently complying with all policies, procedures and guidelines of the company.

§ Complies with company dress code policy.

§ Maintains efficient flow of agency work by reporting to work at assigned or scheduled times; follow procedures for notification of absence or time off requests.

§ Maintains professional /technical competency and proficiency of job responsibilities by attending and participating in meetings, conferences and educational in-service programs as requested.

§ Complies with agency requirements regarding timely completion of company required paperwork.

§ Accepts other duties as assigned.
