RESPONIBILITIES:

• •Use critical thinking skills and respond quickly and professionally under stressful and sometimes emergent situations
• •Provide clinical assessment and nursing triage to a wide variety of patients across the health continuum and/or health education to culturally diverse populations
• •Assists in directing patient to the most appropriate level of care
• •Provide telephonic health information and nursing education
• •Assists and empowers patients to make better health care decisions
• •Helps patients to participate in other external programs or resources available to them to improve their health
• •Work in collaboration with Telehealth providers by referring appropriate patients who may benefit from a “virtual care visit”
• •Provide outbound follow-up calls as necessary
• •All other duties as assigned

QUALIFICATIONS:

Education:

Nursing Degree (Diploma, ADN, BSN) required

Certification and Licenses:

• •Active, unrestricted RN licensure in the state where the candidate resides with the ability to obtain multi-state licensure is required
• •eNLC preferred or ability to obtain

Previous Experience:

• •Two to four years of clinical experience required
• •Hospital experience on medical-surgical, critical care, or inpatient care setting preferred
• •Experience in Emergency Room, EMT, or Triage experience

Knowledge, Skills, and Abilities:

• •Must be a team player
• •Excellent verbal and written communication skills with all members of the healthcare team (nurses, physicians, care managers, social workers, etc.)
• •Understand health insurance, community resources, and other areas associated with coordinating community-based healthcare
• •Excellent organizational skills, can prioritize and complete tasks when there are multiple competing priorities
• •Decisiveness and attention to details as well as proficiency with technology, including computers, software applications, phone systems, etc.
• •Knowledge of Hospital Electronic Medical Records (EMRs)
• •Manage patient information according to HIPPA guidelines
• •Ability to plan, supervise, and review the work of clinical and non-clinical staff
• •Ability to work under stressful conditions or irregular hours

Schedule :

Part-time Day Shift: 7am - 3pm CT / 3 days a week

Pay Range:

27.00 - 40.00 USD per hour This range represents the anticipated base salary for this role. Actual compensation will be determined based on experience, qualifications, and internal equity considerations.
