Job Description:

Position Summary

Unit C3 is the leader of patient progression on the unit, coordinating progression and discharge activities to align with each patient’s target length of stay. In this role, the Unit C3 serves as the primary liaison between interdisciplinary team members on the unit such as the Unit Charge Nurse, Utilization Review nurse, Social Worker, Primary Nurse, PT, OT, and Primary Physician.

Key Responsibilities:

Acts as a spoke of the Hub by partnering with other Care Coordinators to facilitate patient progression and care coordination at a unit level to achieve system goals.

Collaborates with the Hub to ensure appropriate patient placement and level of care (ICU, Telemetry, Med/Surg, etc.).

Evaluates discharge needs upon admission, ensuring each patient has a complete discharge plan.

Works collaboratively with interdisciplinary team to progress each patient's care towards discharge while assuring quality standards and excellent patient experience.

Leads SNAP rounds on the unit with the physician to discuss patient progression and barriers to discharge.

Communicates with patients and families as needed on post-acute plan of care

Identifies potential discharges and updates patient status and planned discharge time

Works with primary nurse and SW on discharge needs/barriers for all patients; initiates intervention by UM nurse as needed

Works with unit staff to ensure Care Progression technology accurately reflects unit and patient status (anticipated transfers, discharges, and patient alerts)

Proactively monitors patients in observation or outpatient status to ensure timely discharge or status change

Communicates service event and patient preparedness issues to Order Coordinator

Ensures acknowledgement and completion of prep prior to testing and/or procedures

Identifies opportunities to transition patient care or testing to an outpatient setting

Assists Clinical Documentation Specialist in engaging physician to respond to queries clarifying documentation to reflect appropriate severity of illness

Serves as a consultant and educator with medical staff regarding status, placement, payor requirements, and utilization of resources

Experience

5 years current acute care hospital experience preferred.

Broad clinical expertise to evaluate patient diagnosis and treatment plan and outcomes utilizing established criteria.

Effective communication, customer and public relations skills.

Ability to work as an integral member of the interdisciplinary assessment team.

Strong organization skills.

Knowledge of financial reimbursement, state, federal and commercial insurance requirements preferred.

Knowledge of community resources preferred.

Acute hospital case management experience preferred.

Utilization Management experience preferred

Education

Bachelor’s degree in nursing preferred.

Must pass orientation and initial competency assessment prior to independent assignment of patients requiring the applicable skill, and must complete and pass all annual competencies.

Licensure/Certifications

State of California RN license required. American Heart Association Healthcare Provider BLS required.

Pay Range (based on years of applicable experience):

$73.13 to

$97.81

The hours employees work determine when a shift differential is paid.

Hourly Evening Shift Differential: $5.50

Hourly Night Shift Differential: $8.00
