Job Summary

The Housing and Case Management Supervisor oversees the case management framework, housing navigation, and supportive service coordination for single adult residents aged 25 and older at KLEO’s ELEVATION House. Operating within a trauma-informed, harm-reduction, and Housing First framework, this salaried supervisory role directs and coaches Youth Client Services Specialists, drives individual housing stability outcomes, and fosters community collaborations to assist single adults in transitioning successfully from emergency shelter to stable, permanent housing.

Key Responsibilities

Case Management & Staff Supervision

Directly supervise, mentor, and evaluate Youth Client Services Specialists assigned to the adult single-resident program at ELEVATION House.

Conduct regular individual and team supervision sessions to review case progress, audit service delivery, and guide interventions for complex adult client barriers.

Maintain a reduced, specialized direct caseload (approximately 10–12 single adult residents facing significant behavioral health, medical, or chronic housing barriers).

Oversee comprehensive intakes, vulnerability assessments, and individualized housing stabilization plans completed by direct reports.

Ensure all client progress notes, service plans, incident documentations, and exit records are accurately maintained in internal databases and the Homeless Management Information System (HMIS) within prescribed 24- to 48-hour standards.

Facilitate weekly case review meetings with Youth Client Services Specialists to identify service delays, review housing applications, and refine barrier-reduction strategies.

Housing Navigation & Supportive Services

Lead site housing location, application, and placement efforts through the local Coordinated Entry System (CES), Chicago Housing Authority (CHA) programs, rapid re-housing funds, and permanent supportive housing (PSH) networks.

Guide staff in connecting adult residents to mainstream benefits (SSI/SSDI, SNAP, Medicaid, TANF, Veterans benefits) and vocational/workforce training resources.

Cultivate working relationships with local property owners, landlords, behavioral health clinics, substance use treatment providers, and healthcare facilities.

Organize adult-focused on-site workshops covering tenancy readiness, budgeting, and lease compliance.

Support & Safety Coordination

Collaborate closely with the Shelter Support Supervisor to maintain seamless communication regarding daily room turnover, census verification, and resident welfare checks.

Provide secondary de-escalation and crisis management support when behavioral or mental health escalations occur on the floor.

Review critical incidents involving residents on assigned caseloads, document clinical follow-up actions, and submit reports to the Project Manager of Client & Community Services.

Administrative & Grant Compliance

Perform regular quality-assurance file audits on direct reports' client records to ensure adherence to agency standards and municipal funder guidelines.

Compile monthly performance, occupancy, and housing placement outcome data for submission to the Project Manager of Client & Community Services.

Participate in agency leadership sessions, required compliance trainings, community partner meetings, and organization-wide events.

Perform all other duties as assigned.

Qualifications & Requirements

Education & Experience

Bachelor’s degree in Social Work, Human Services, Psychology, or a related behavioral science field required (Master’s degree / MSW preferred).

Minimum of 3 years of direct case management experience in housing, homelessness services, or supportive social services.
