• •Responsible for establishing, implementing and evaluating a plan of care to restore function and prevent disability following disease, injury or loss of a body part.

• Education
• •Masters Degree in Physical Therapy or Bachelor’s degree in Physical Therapy with a minimum 5 years of work experience.

• Licenses and Certification
• •Illinois license in Physical Therapy
• •BLS

• Experience and Skills
• •Technical Experience: N/A

• Role Specific Responsibilities
• •Performs thorough evaluation
• •Develops appropriate Plan of Care
• •Implements Plan of Care
• •Documentation is accurate, complete and timely
• •Provides supervision to assistants, rehab techs, and students

Compensation (Commensurate with experience):
