Your Responsibilities:

• •Assist Physical Therapists in establishing and administering treatment plans with specific therapy goals.
• •Help assess patient needs; plan, evaluate, and modify care to meet physical therapy intervention goals.
• •Collaborate with all disciplines to plan and evaluate team goals for each patient.
• •Support in setting realistic and achievable goals for patients/residents.
• •Document and record patient/resident condition and progress per state and federal regulations.
• •Educate patients and families in appropriate therapy methods.
• •Optimize patient functionality and skills by assisting in the screening of all residents.
• •Communicate patient status and needs to the patient, family, and other involved professionals.
