Ihr Profil:

  • Abgeschlossenes Studium als Sozialarbeiter/in oder Sozialpädagoge (m,w,d) oder vergleichbare Qualifikation
  • Gültige Fahrerlaubnis der Klasse B
  • Gute EDV-Kenntnisse Einfühlungsvermögen, Beratungskompetenz und Konfliktfähigkeit
  • Eigenständiges und strukturiertes Arbeiten
  • Erfahrungen mit psychisch kranken Menschen wünschenswert, aber nicht Bedingung

Ihre Aufgaben:

  • Aufsuchende Sozialarbeit im Sinne der sozialen Integration
  • Intensive, individuelle am Hilfeplan orientierte Einzelfallbetreuung
  • Gesamtes Antragverfahren gemäß SGB IX inklusive Hilfeplanerstellung
  • Erstellung von Dokumentationen, Berichten und Betreuungsstundennachweisen
  • Teilnahme an Hilfeplankonferenzen
  • Begleitung und Koordination bei Behörden, ärztlichen Terminen und Austausch mit anderen Stellen im Helfersystem
  • Krisenintervention, beratende / entlastende Gespräche, Begleitung bei Klinikeinweisungen
  • Unterstützung der Klienten in alltagspraktischen Fragen wie Haushaltsführung, Finanzen und Gestaltung des eigenen Wohnumfeldes
  • Familiengespräche
  • Regelmäßige Teilnahme an Dienstbesprechungen und Supervision
  • Kooperation mit dem Sozialamt, Jugendamt bzw. anderen beteiligten Institutionen und Organisationen
