About the Position
In collaboration with the multi-disciplinary Reid Hospice team, the Hospice Case Manager RN is responsible for coordination of all care for their assigned hospice patients; this includes admission, discharge, re-certification, and planning of care. The Case Manager RN will work closely with the Hospice Interdisciplinary Group (IDG), including patients and families, as well as Reid Health Leadership, Medical Staff, and physician’s practice office personnel; will work with representatives of the community, federal, and state agencies, as well as with outside review and accreditation agencies.

Must maintain competence in BLS after initial training.
Expected to demonstrate a calm, pleasant, and professional demeanor.
Must have the ability to work cooperatively with all staff, physicians, and departments.

Education & Experience
Minimum education required: Associates, Bachelor's, or Masters Degree in Nursing
Licensure & Certification
Minimum licensure required: Licensure in State of Indiana as a Registered Nurse
