BMT Program Coordinator

Job Description Summary:

Ensures care continuity for patients and families and collaborates in the development, implementation, and evaluation of all program components.

Job Description:

• Essential Functions:
• •Develops, implements, and enhances special projects promoting inpatient/outpatient continuity of care and parent engagement.
• •Ensures the provision of high quality, individualized, comprehensive, cost effective patient across the continuum.
• •Initiates, tracks, analyzes, and manages all program quality improvement and research projects, program goals and objectives.
• •Coordinates functions with interdisciplinary medical care, liaison functions with patient, family, medical staff, and referring physicians.
• •Develops educational materials and programs for patients, nursing and ancillary staff, and physicians.
• •Develops clinical protocols, policies, and procedures including community education and public relations.

• Education Requirement:
• •Bachelor's Degree, required.
• •Masters Degree in Nursing, preferred.

Licensure Requirement: Registered nurse in the State of Ohio, required.

• Skills:
• •Effective customer service skills.
• •Good multiplatform communication skills (oral, written, presentation).
• •Ability to assist in conflict resolution.
• •Problem-solving and interpersonal skills required for interaction with contacts required.

Experience: Three years of registered nurse experience, required.
